You are eligible for Health and Life Benefits coverage if you are on the U.S. payroll as a full-time employee scheduled to work at least 30 hours per week. Dependents eligible for coverage under Health and Life Benefits include:
Your spouse/domestic partner is eligible for First Data's medical plan only if he/she is not eligible for coverage through his/her employer (regardless of whether or not he/she is enrolled in that coverage). You must actively enroll him/her in coverage. You will also need to complete dependent verification -- watch your home mail for this important information. First Data periodically verifies spouse/domestic partner eligibility.
You will need to re-enroll your eligible spouse/domestic partner in medical coverage every year unless indicated otherwise. If your spouse/domestic partner is eligible for First Data medical coverage, you must complete the online certification and enroll him or her in coverage. If you do not take this step each year, your spouse/domestic partner may be dropped from coverage!
This exclusion does not apply when you and your spouse/domestic partner both work at First Data. The exclusion is only for medical coverage and does not impact eligibility for dental and vision coverage.
Be sure you and each of your dependents enrolled in a medical or dental plan have a valid Social Security number (SSN) on file in Your Benefits Resources. Dependents without an SSN on file cannot participate in First Data benefits.
When you enroll any dependent in coverage for the first time, you will receive information regarding dependent verification, a process employees where employees must provide specific documentation that validates dependents are eligible for medical, dental and vision coverage. If you do not comply with the dependent verification requirements, your dependents will be dropped and will not have First Data benefits. Employees who have enrolled ineligible dependents will be subject to disciplinary action up to and including termination of employment.
