Summary Annual Reports

First Data is required to provide all plan participants and beneficiaries with the Summary Annual Report (sar) for the company’s Health & Life Benefits plans. The sar summarizes the financial information that was provided in the annual Form 5500 and accompanying Schedules, also referred to as the annual report.

The sar includes administrative expenses incurred by the plan, amount of benefits paid to participants and beneficiaries, total value of plan assets and the right to receive a copy of the full annual report.
