Qualified Family Status Change

You must experience a qualified family status change to modify, add or drop your Health and Life Benefits during the plan year. If the change causes a change in eligibility for you, your spouse or eligible dependents, you may be eligible to change your coverage, except LTD and the Legal Plan. The deadline for submitting a family status change request is 31 days from the date of your event, except for births and adoptions, which have a 90-day deadline.

  • Birth/adoption
  • Marriage
  • Divorce
  • Loss of dependent eligibility (certain events)
  • Dependent gain of eligibility
  • FT to PT change of hours
  • PT to FT change of hours
  • Spouse gain of employment
  • Spouse loss of employment
