Word to the Wise: When you're young and healthy, life insurance is one of the last things on your mind. But it's actually the best time to buy coverage because you'll pay less than if you wait until you're older.
Life and Accident Insurance
Life and Accident insurance protect you, your spouse and your dependents in the event of death or certain types of injuries resulting from an accident. First Data provides some coverage at no cost to you, and you can purchase additional coverage when you enroll in Health and Life Benefits. Review the 2012 Summary Plan Description for complete details.
Life Insurance
- Basic Life Insurance pays a benefit equal to your annual benefits salary, up to a maximum of $1 million, in the event of your death. You are enrolled automatically for this coverage, and First Data pays the full cost.
- You pay the cost of voluntary life insurance, and you choose the amount(s) when you enroll:
- Supplemental - choose from 12 coverage options
- Spouse - choose from 11 coverage options
- Child - either $5,000 or $10,000. All enrolled children are insured at the same level. You pay one price no matter how many children you elect to cover. You must designate the eligible child(ren) you will cover on Your Benefits Resources
About Evidence Of Insurability (2011)
You need to provide proof of good health—also called EOI—in the following cases:
If you want to purchase Supplemental Life Insurance AND you meet one of the following criteria:
- You previously waived coverage and want to elect coverage for 2011.
- You currently have coverage and want to increase your coverage.
If you want to purchase Spouse Life Insurance AND you meet one of the following criteria:
- You previously waived coverage and want to elect coverage for 2011.
- You currently have spouse coverage and want to increase coverage.
If EOI is required for your Life Insurance, once you have successfully completed your enrollment online, you will see a message on your Completed Successfully page requesting you to complete your MetLife EOI form online. You will automatically be processed in MetLife’s system without mailing any information back to MetLife.

Accidental Death & Dismemberment (AD&D) Insurance
- Basic AD&D insurance pays a benefit equal to your annual benefits salary, up to a maximum of $1 million, in the event of your death or certain types of injuries resulting from an accident. You are enrolled automatically for this coverage, and First Data pays the full cost.
- Voluntary AD&D insurance pays a benefit amount that you choose when you enroll. Benefits are paid if you, your spouse or your child(ren) die or suffer certain types of injuries as the direct result of an accident. The maximum benefit is up to 10 times your annual benefits salary to a maximum of $1 million.
Business Travel Accident Insurance
Business Travel Accident insurance pays a benefit of up to four times your annual benefits salary in the event of your death or certain types of injuries suffered while you are traveling on First Data business. The maximum benefit is $1,000,000 (subject to certain exclusions). For more about this coverage, including exclusions that apply, read the 2012 Summary Plan Description and the AIG assist brochure.
Beneficiary Designation
To designate or change your beneficiary for Life and AD&D insurance, go to Your Benefits Resources, and click on Topics on the left, and then on Beneficiaries.
